Risk Management in Anticoagulation

In this knowledge-based continuing pharmacy education activity Dr Michael Smith, PharmD, CACP, BCPS discusses the main issues related to the management of risk when pharmacists are involved in anticoagulation management.

Target Audience

Pharmacists who are interested in the management of patients on anticoagulation therapy and/or who are interested in starting pharmacy services for patients on anticoagulation therapy.
This activity is not accredited for technicians.

Learning Objectives

At the conclusion of the activity, pharmacists will be able to:

  • Discuss the education and training needs of pharmacists who participate in anticoagulation services.
  • Discuss the documentation needs of a pharmacists-run anticoagulation service or clinic.
  • Identify corporate infrastructure needs to support anticoagulation services or clinics.
